Please note that a limited number of FAI certification course registrants are accepted due to the hands-on nature of these courses.

Payment Details:

There are four payment options available-

1. PayPal/Credit Cards: Credit Card Payments can be securely completed online. After completion of the registration form below (and clicking the Submit icon), you will be presented with the option to pay online via Paypal.

(Please be aware that Credit Card Payments will be processed via our affiliate F.I.R.S.T. Health and as such will appear on your Credit Card billing statement.)

2. Fax Option:You may also fax the below completed form to our fax number as listed at the bottom of this page - please note the form may not be accessible in some browser types. Download registration form and fax completed form to FAI (click the following link to download the form)
